Our Home Support Service provides companionship and invaluable support to people living in their own homes with dementia across Wiltshire. The much-valued service won Best Dementia Team at the National Dementia Care Awards in 2018.

We are looking for a Care Coordinator to run our well-established service in the Salisbury area. You’ll get to know the families well, and manage a team of up to ten support workers who spend quality time with clients in two or three hours visits. It’s a chance to really make a difference in a forward-thinking, award-winning organisation.

You will:

  • Coordinate our Home Support Service to all existing clients within the area
  • Process new referrals and set up new services with new clients
  • Ideally have experience of completing care plans, risk assessments and reviews
  • Be involved in the recruitment, training, supervision and management of the staff team
  • Enter and maintain accurate data on client database, with competent computer literacy skills
  • Communicate confidently and effectively

Hours: 18 hours per week

Salary: £21,916 per annum pro-rata (Scale points 24-26)
